Dr. Toby Meltzer

Dr. Toby Meltzer, MD is a plastic and reconstructive surgeon who is globally recognized as one of the leading surgeons in the field of Gender Affirming Surgery. Dr. Meltzer began performing Gender Affirming Surgery procedures in the early '90s and has experience with FTM Surgery that only a select group of surgeons in the world can claim. Dr. Meltzer has offices in Scottsdale, Arizona and Portland, Oregon.
Dr. Meltzer became the first surgeon in Arizona to specialize in Gender Surgery when he moved from Portland to Scottsdale to expand his practice 2003. However, several key staff members from The Meltzer Clinic stayed on in Portland, and Dr. Meltzer has always had a large number of patients based in the Pacific Northwest. To improve accessibility for these patients, Dr. Meltzer and Dr. Ley recently partnered with Legacy Good Samaritan to offer FTM Surgery in Portland once again. Patients can choose between either Scottsdale or Portland for surgery dates.
Education, Training & Memberships
- Graduated from LSU Medical School in 1983
- Completed his plastic surgery residency at the University of Michigan
- Board-certified in both General and Plastic Surgery
- Served as an Assistant Professor of Plastic and Reconstructive Surgery at Oregon Health Sciences University (OHSU) from 1990 to 1996, before going into private practice.
- Member of the American Society of Plastic and Reconstructive Surgeons and the World Professional Association for Transgender Health (WPATH).

FTM Surgery in Arizona & Oregon
Dr. Meltzer offers the following masculinizing procedures:
- Top Surgery: Double Incision and Peri-Areolar
- Simple Release Metoidioplasty and Full Metoidioplasty, which includes Vaginectomy (performed by Dr. Burt Webb, who can also perform Hysterectomy), Scrotoplasty and Testicular Implants. Dr. Meltzer can also perform Mons Resection to optimize Metoidioplasty results.
- Phalloplasty: Groin Flap, RFF, Combined Groin + RFF, incl. Urethroplasty, Glansplasty, Penile Implant
- Facial Masculinization Surgery
Surgery Requirements
Dr. Meltzer follows WPATH guidelines for surgery:
- You will need to provide two referrals from qualified mental health professionals for genital procedures; one referral for other procedures unless otherwise specified by insurance.
- A letter from your hormone specialist stating you have been on 12 months of continuous hormones for genital procedures, unless clinically contraindicated.
- Dr. Meltzer will operate on minors with parental consent.
Financing & Insurance
Dr. Meltzer accepts cash, credit cards and medical financing. He contracts with Kaiser and has also worked successfully with BCBS, UHC, Aetna, Cigna and Harvard Pilgrim insurance. Dr. Meltzer also accepts Medicaid (Apple Health.)

NOTE: Dr. Meltzer is a regular presenter at transgender conferences, including First Event, Gender Spectrum, Gender Odyssey, Keystone, and Southern Comfort. Dr. Meltzer often does consultations at conferences, offering a convenient way for patients from around the country to meet with him without having to travel to Arizona or Oregon for a consult.
