What sort of questions should I ask? I've been researching this for years but I do not want to forget anything.

I'll add to this: ask about nipple placement. Look at photos to get an idea of what you'd like for placement and compare to the surgeon's result photos. Make sure you're on the same page about nipple size and placement.
