"Dog ears" are excess skin. Whether or not someone develops dog ears after surgery is mostly in the hands of the surgeon, and surgeons who do a lot of top surgeries are getting better and better at avoiding them. The best thing you can do is maintain a healthy weight, as the heavier one is the more skin there is. Also, be patient after surgery. Puffiness and swelling can look like they might develop into dog ears, but it takes several months for the chest shape to settle.

If you do develop dog ears, revision surgery is a fairly simple affair. Review your surgeon's revision policy carefully to find out if revisions are included in the initial cost of surgery. (They were included with my surgeon.)
