Insurance approval - Jai'son - 05-02-2014
Can anyone tell me what NEEDS to be said in my letter for Top surgery? My insurance will cover it, but I have to get a letter from my therapist or counselor. I have been looking all over and I can't find anything. Is there a place I can look at sample letters to see what mine needs to say.
