In Kansas, there's Dr. Gutek near Overland Park, and Dr. Lovelle in the Wichita area. http://www.topsurgery.net/states/kansas.htm Dr. Gutek is around $6000, not sure about the cost with Dr. Lovelle.

I think Denver may be even closer, and I'm pretty sure Dr. Steinwald has much more experience than Drs. Gutek and Lovelle.
